DVLA and taxi medicals in Ilkley

If you drive lorries or buses, or need C1 or D1 on your licence, you'll need a D4 medical:

  • when you first apply

  • at age 45

  • every five years until you're 65

  • every year after that

Our GMC-registered doctor completes your D4 at a face-to-face appointment at our Ilkley clinic. It costs £90, with no hidden fees.

Required Documents

After your medical

Your completed D4 is valid for four months from the date it's signed. Send it to the DVLA with your application within that time.

If you need more tests

If your medical shows you need further tests, we can often do them here, which saves waiting. At extra cost, we offer:

Taxi and private hire drivers

We also carry out taxi and private hire medicals. Each council has its own medical form, and some, including Leeds and Bradford, say the doctor must have full access to your medical records. Please send us a copy of your full medical record from your GP practice before your appointment, or bring it with you, along with any forms that need filling in. GP practices can take up to a month to provide a copy, so ask in good time. Without these we can't carry out your medical, and we can't give a refund if you arrive without them.
